ABSTRACT

BACKGROUND: Many researchers at home and abroad think that interlocking intramedul ary nails may be more suitable for comminuted fractures of the tibia with severe soft tissue injury. Plate fixation is more advantageous for distal tibial fractures. OBJECTIVE: To compare the effects of multifunctional locking intramedul ary nail and locking plate in treatment of extraarticular proximal tibial fractures. METHODS: 156 patients with extraarticular proximal tibial fractures were included and divided into intramedul ary nail group (n=78) and the locking plate group (n=78) according to repair method. Curative effects and healing time were compared between the two groups. At 3 months after treatment, Johner-Wruh tibial fracture scores were used to assess the repair effect in both groups. In addition, operation time, blood loss, healing time and complications were compared between the two groups.RESULTS AND CONCLUSION: Mean healing time was 9.3 months in the locking plate group and 9.2 months in the intramedul ary nail group. At 3 months after treatment, the repair effect was significantly better in the intramedul ary nail group than in the locking plate group (P < 0.05). Blood loss and time were less in the intramedul ary nail group than in the locking plate group (P < 0.05), and surgical difficulty was high. At 3 months after treatment, the incidence rates of tibia eversion/inversion, neurovascular injury and nonunion/infection were significantly lower in the intramedul ary nail group than in the locking plate group (P < 0.05). These findings confirmed that the multifunctional locking intramedul ary nailing for extraarticular proximal tibial fractures obtained reliable efficacy, exact effect, reduced blood loss and low incidence of complications. Thus, it is better than locking plate.

ABSTRACT

Objective To draw for character effective sports therapy for middle‐age patients with type 2 diabetes .Methods Choosing 16 middle‐aged patients with type 2 diabetes suitable for sport ,testing the VO2 max ,fasting plasma glucose and the differ‐ent time points after meals ,different intensity exercise corresponding heart rate to find out effective exercise intensity and blood glu‐cose peak time .To determine patients the best starting time ,carrying out effective intensity and time exercise 15 ,30 ,45 ,60 ,75 min before the peak time .Results (1)The blood glucose significantly reduces that 15 min low intensity and 5 min medium intensity were compared with pre‐movement(P0 .05) .(3) Glucose‐lowering effect was no difference of the movement at different time point before the peak ,but 15-45 min before the peak time was better .Conclusion The middle‐aged patients with type 2 diabetes should combine their own blood glucose peak time ,proceeding moderate intensity more than 15 min or low intensity more than 45 min ,they can achieve good hypoglycemic effect at 15-45 min before the peak time .

ABSTRACT

<p><b>OBJECTIVE</b>Sr-HA, a new type of hydroxyapatite biomaterial, was implanted into animals to study the bioreaction and character, which would be helpful for the further clinical applications in the future.</p><p><b>METHODS</b>Totally 24 rabbits were divided into 3 groups. The bone defect of 6 mm x 12 mm x 4 mm was made at both mandibular angles of rabbits and Sr-HA of different proportion (10%, 5%, 0) was applied to reform the defects. One group of animals were killed randomly at 1, 3 and 6 months after operation to evaluate the material biological compatibility using anatomic, X-ray examination, histological and ECT methods.</p><p><b>RESULTS</b>The histological photographs showed that Sr-HA caused little infection around implanted area and, almost was not repulsed by hosts. With the degradation of biomaterial, there was more apparent new bone growth in the area around Sr-HA than that around HA and some ossification can be found in soft tissue nearby. Also a tight osteointegrity was gradually got after the operation, according to the results of X-ray and, the border between Sr-HA and bone was hardly discovered at the 6th month after the operation. A more obvious nuclide assembling was observed at the side of Sr-HA by ECT images. With the biodegradation of Sr-HA, more new bone was intruded into the spare space of the biomaterial.</p><p><b>CONCLUSION</b>Sr-HA has better biocompatibility and higher biodegradation than that of pure HA. It holds an excellent osteoinductivity and fair osteoconductivity to some degree too. So a more satisfying effect of bone defect rehabilitation was gained with the increasing new bone depositing in the free space of the material, when it degraded gradually.</p>

ABSTRACT

Objective To know the accumulation and movement of heavy metals in the environment through sewage sluge using in agriculture. Methods The sewage sludge samples were collected from sewage treatment farms of Suzhou, Wuxi and Changzhou in the different seasons of 1998. The content and states of heavy metals were determined by AAS. The accumulation and movement of heavy metals were analyzed as sewage sludge was used in agriculture. Results The content of heavy metals in the sewage sludge was greatly higher than that in the soil background. Especially, the contents of zinc in the sewage sludge in three cities exceeded the related standard, the means were 1 199.00, 784.00 and 1 216.00 mg/kg respectively. A long term using of sewage sludge might induce heavy metal accumulation in the soil. Conclusion Sewage sludge used in agriculture may induce heavy metal accumulation in the soil and the organisms, so the risk of environmental heavy metals accumulation may increase.
